While clinicians want to do well by patients and families, they often lack necessary knowledge and skills to honor previous therapeutic relationships and successfully transition patients to the Adult hospital atmospher. Likewise, receiving adult practitioners may lack an understanding of the previous therapeutic experience the patient has endured. In this session, the complex emotions and decision making roles established between young adults, their parents and their providers will be discussed. .This session incorporates both the evidence and practical advice from extensive experience.
