Hemoadsorption is a treatment whereby solutes are removed extracorporeally by direct binding to the sorbent material. The rationale for its use is that it can alter the immunologic burden of several diseases. It has been applied to several critically ill patients although its benefits remain uncertain. This pro/con debate will review the physiologic rationale and the current state of evidence for hemoadsorption as a therapy in critically ill patients.
