Multidisciplinary and Patient-Centered Care: The ICU a Natural Access Point?

Using congenital heart disease and cystic fibrosis as two model chronic illnesses for transition, the challenges and successes in transition will be discussed. Likewise, further discussion will include what every Pediatric cardiac intensivist would like to say to their adult counterparts about taking care of adults with CHD in the hospital and beyond.
