Acute respiratory failure is both a common indication for definitive airway management in the intensive care unit and an important factor that limits the efficacy of traditional approaches to preoxygenation. Evidence-based approaches to optimal preoxygenation and other means by which to temporize hypoxemia during airway management will be reviewed.
