Whether in pediatrics or adults, the microbiome is gaining understanding in its effects on critical illness.  There are many questions that have come out of research:  Does the microbiome cause critical illness? Does the microbiome modulate outcome in critical illness? Can the microbiome be harnessed to protect against critical illness?  These questions will be answered and further details about the microbiome will be discussed:  microbial niche establishment, virulence factors, regulation of gene expression, state of health immunologic competence, virulence factor susceptibility, and tissue-specific regulation of microbiome function.
