Extracorporeal life support (ECLS), including various modalities of extracorporeal membrane oxygenation (ECMO), has emerged as a core intervention in critical care medicine, particularly during the H1N1 and COVID-19 pandemics. The complex physiology of ECMO can have a profound effect on medication management, including expanded volumes of distribution, altered renal perfusion, and drug sequestration in the circuit. In this presentation, we will review the major considerations regarding drug therapy for the patient receiving ECMO, including antibiotics, sedatives, and anticoagulation considerations.
