(386) Association of Continuous Renal Replacement Therapy With Vitamin Deficiencies in ICU Patients

Introduction: Given the significant morbidity and mortality associated with malnutrition, identification of malnourishment and specific vitamin and micronutrient deficiencies in critically ill patients is crucial to earlier intervention and improving patient outcomes. Patients undergoing continuous renal replacement therapy (CRRT) are thought to be associated with an increased risk of malnutrition and vitamin or micronutrient deficiencies, although data remains limited in this population. We hypothesized that patients on CRRT for longer units of time would have a higher rate of documented vitamin and/or micronutrient deficiencies.

Methods: An IRB-approved retrospective review was performed at our large academic hospital, with data collected on 117 critically ill inpatients between January through September 2022 requiring CRRT, including rates of malnutrition (per EMR or Dietician Coding) and the diagnosis of vitamin or micronutrient deficiencies. Simple logistic regression was utilized to analyze data to determine rates of malnutrition and documented vitamin deficiencies in this population.

Results: Of the 117 patients on CRRT analyzed, malnutrition was documented in 40.2% (47 patients). For every day that a patient remained on CRRT, there was an 8.5% increased risk of a documented vitamin deficiency (p-value = 0.0005).

Conclusions: Patients on CRRT have a high rate of malnutrition diagnosis and are noted to have a time-related, increased risk of developing a documented vitamin deficiency. Further investigation is required to characterize the specific vitamin and micronutrient deficiencies that patients on CRRT acquire, determine ideal timeframes for analyzing vitamin and nutrient levels, as well as to develop optimal repletion algorithms for this patient population.
