(1233) Enhancing Handoff Process in Critical Care Training Program: Integration of SmartPhrases in EMR

Introduction: Effective handoff communication plays a crucial role in ensuring patient safety and continuity of care in critical care settings. The adoption of electronic medical record (EMR) systems, such as EPIC, has revolutionized healthcare documentation, including the handoff process. This study explores the implementation of SmartPhrases, an innovative feature within the EPIC EMR system, to improve handoff efficiency and quality within a critical care training program.

Methods: SmartPhrases are pre-defined templates that allow clinicians to input standardized information rapidly and accurately. The templates were designed to capture essential data points and critical care-specific information, ensuring a comprehensive and consistent handoff report. The study included a pre-intervention assessment of handoff practices, followed by the introduction of SmartPhrases into the EPIC EMR system for use during handoff communication. Post-intervention evaluations were conducted through surveys, observation, and EMR queries to gauge the perceived effectiveness of SmartPhrases in improving the handoff process. The process involved Education on handoff process and SmartPhrases coupled with weekly reminders to all involved staff.

Results: A cohort of 14 critical care fellows participated in this quality project. Participants reported increased daily updating of handoff from 71 % to 91 % (p = 0.04) and improved accuracy and communication from 61 % to 95 % (p = 0.001). Common hurdles identified in relation to the daily updating of handoff information include: time Constraints, information overload, and integration with daily workflow.

Conclusions: The integration of SmartPhrases in the EMR system positively impacted the handoff process in the critical care training program. Participants reported increased efficiency, improved accuracy, and enhanced communication during handoffs. The use of standardized templates facilitated the exchange of critical patient information and reduced the risk of information loss or miscommunication. Further research is warranted to explore and develop strategies that address the identified hurdles in order to optimize the daily updating of handoff information and enhance the overall effectiveness of the handoff process.
