(314) Delirium in Patients Undergoing CABG or AVR: A National Inpatient Sample Analysis for the Year 2020

Introduction: Delirium is a common complication in hospitalized patients, and understanding its association with demographic and clinical factors is crucial for improving patient outcomes. This study aimed to investigate the prevalence of delirium in patients undergoing coronary artery bypass grafting (CABG) or aortic valve replacement (AVR) and identify potential risk factors.

Methods:

Methods: The NIS data for 2020 was analyzed, encompassing a sample of 35,722 patients who underwent CABG or AVR, drawn from a large cohort of 6,471,165 hospital admissions. Patients with cell counts less than or equal to 10 were excluded, as mandated by the Healthcare Cost and Utilization Project (HCUP).

Results:

Results: Among the 35,722 patients, 840 (2.35%) experienced delirium during their hospitalization. The analysis revealed that patients aged 65 years and above had a significantly higher risk of developing delirium compared to those aged 15 to 40 years (adjusted odds ratio [aOR] = 2.849, p = 0.042). Additionally, patients with congestive heart failure (aOR = 1.5, p < 0.001), peripheral vascular disease (aOR = 1.21, p = 0.038), cerebrovascular disease (aOR = 1.213, p = 0.059), and dementia (aOR = 4.879, p < 0.001) had increased odds of delirium. Length of stay was also a significant factor, with a length of 14 days or more associated with a higher risk of delirium (aOR = 3.829, p < 0.0001).

Conclusions:

Conclusion: This study provides insights into the prevalence and risk factors for delirium in patients undergoing CABG or AVR during the year 2020. Advanced age, congestive heart failure, peripheral vascular disease, cerebrovascular disease, and dementia were identified as significant risk factors for delirium. Hospital region, length of stay, and insurance type also influenced delirium incidence in this patient population. These findings can assist healthcare providers in identifying at-risk patients and implementing targeted interventions to improve patient outcomes. Further research is warranted to explore underlying mechanisms and develop preventive strategies to reduce delirium rates in this population.
