(1083) Assessment of an International Critical Care Education Program Alumni: A Longitudinal Follow-Up

Introduction: The Checklist for Early Recognition and Treatment of Acute Illness and Injury (CERTAIN) initiative is a global critical care quality improvement project that has shown to improve processes of care and patient outcomes. To increase the dissemination of the CERTAIN approach, an interactive virtual course was designed. The aim of this study is to evaluate the variability in the long-term adoption between learners and investigate potential barriers to the implementation process.

Methods: A 20-question online survey was designed and distributed to the CERTAIN leaners via the Mayo Clinic Survey Research Center on 7/27/2023. Dillman’s method with corresponding modifications for electronic delivery was employed. The questionnaire aimed to evaluate various aspects of the implementation process alongside with capturing demographic and workplace characteristics. Free text comment options were also provided. Herein we report the interim analysis of this ongoing survey study.

Results: A total of 57 responses were received, with 76% physicians with over ten years of experience, and 63% working on an ICU. The range of the number of ICU beds in participants’ institutions was 13 to 35, and 64% had with fewer than 10 ICU physicians. 77% of participants took the course within the last five years, and their enrollment was either in the online version (75%) or the year-long coaching program (31%). Regarding the use of the CERTAIN approach checklists, most of the participants report using the admission checklist some days (33%) and the rounding checklist most days (32%). 36% stated that they never use the Get To Know Me Board, despite the fact that the majority claimed that their ICU had incorporated the material taught in the course (68%). The major barrier shown was the limited completion rate among team members (52%) followed by the lack of the checklist embedded on the electronic health record (32%).

Conclusions: While the transition to a virtual course has expanded the reach of the CERTAIN program, the level of local adoption still varies among hospitals and countries. The barriers identified represent an opportunity to identify measures to facilitate the sustained and successful integration of the CERTAIN program.
