Introduction:

Objectives: Define and map subcompetencies for pediatric cardiac critical care (PCCC) fellowship to an entrustable professional activity-based framework by integrating learning objectives and curricular competencies from established, consensus-based curriculum guidelines and entrustable professional activities (EPAs).

Methods:

Design: A systematic modified Delphi method was used to develop a comprehensive framework based on previously published consensus-based curriculum guidelines and EPAs.
SETTING: Independent expert ratings and repetitive feedback from a volunteer group of PCCC fellowship program directors, followed by presentation and acceptance by PCCC program directors from across North America.
SUBJECTS: Pediatric cardiac intensive care physicians.
INTERVENTIONS: None.

Results: MEASUREMENTS AND MAIN

Results: The procedure for defining this framework consisted of 6 steps completed consecutively: (i) literature search, (ii) extraction of key subcompetencies and curricular components, (iii) written questionnaire, (iv) consensus meeting and critical evaluation, (v) approval by guideline developers, and (vi) broad in-person acceptance. A total of 110 subcompetencies were mapped to 9 EPAs with anchored levels of entrustment and examples of simple and complex cases. To facilitate clarity and the development of a future assessment tool, 3 EPAs were divided into subcategories with related subcompetencies mapped to the appropriate subcategory, the latter primarily encompassing common procedures performed in the cardiac intensive care unit.

Conclusions:

Conclusions: This study describes the systematic, stepwise method used to define 110 subcompetencies mapped to established EPAs for pediatric cardiac critical care. This serves as the foundational step in the integration of EPAs into the development of the first competency-
