(1333) Role of Group 5 Pulmonary Hypertension in End-Stage Renal Disease Patients

Introduction: Pulmonary hypertension (PH) is a known complication in end-stage renal disease (ESRD) patients and can be categorized into different groups based on its etiology. Group 5 PH, characterized by unclear multifactorial mechanisms, poses a unique challenge in ESRD patients. This study aims to examine the impact of Group 5 PH on mortality in ESRD patients and identify other significant predictors of mortality using data from the National Inpatient Sample (NIS) in the year 2020.

Methods: Data from the NIS for the year 2020 were analyzed to identify a cohort of ESRD patients with Group 5 PH (N = 205,660) from a total sample size of 6,471,165. Multivariate logistic regression was performed to determine the adjusted odds ratio (aOR) and associated p-values for various predictors of mortality in ESRD patients with Group 5 PH. The predictors included age group, gender, race, region of hospital, primary payer/insurance, location/teaching status of the hospital, length of stay, and comorbidities.

Results: Among ESRD patients with Group 5 PH, the mortality rate was significantly influenced by various factors. Patients aged ≥65 years had the highest odds of mortality (aOR = 3.22, p < 0.0001), followed by those aged ≥40 to < 65 years (aOR = 1.88, p < 0.0001) compared to the reference group (age ≥15 to < 40 years). Other significant predictors of mortality included moderate/severe liver disease (aOR = 2.61, p < 0.0001), COVID-19 infection (aOR = 4.725, p < 0.0001), metastatic cancer (aOR = 2.276, p < 0.0001), and cerebrovascular disease (aOR = 1.483, p < 0.0001), among others.

Conclusions: Group 5 Pulmonary Hypertension in ESRD patients is associated with increased mortality risk, particularly in older age groups and those with specific comorbidities. Identifying these predictors of mortality can guide healthcare providers in tailoring management strategies for ESRD patients with Group 5 PH. Further research and awareness are crucial to improve outcomes in this vulnerable patient population.
