(1081) A Novel Validated Checklist for Videolaryngoscopy Review

Introduction: Endotracheal intubation of critically ill patients is a potentially life-saving and high risk procedure. Review of recorded video laryngoscopy intubations represents an alternative avenue for high-fidelity airway education and increased procedural exposure. However, despite the widespread use of video laryngoscopy for endotracheal intubation, there is no accepted standardized approach to the evaluation and review of recorded video laryngoscopy. In this study, we sought to enlist a multidisciplinary team of airway experts to create a standardized, reliable, and validated checklist for reviewing recorded video laryngoscopy intubations.

Methods: After a literature review, a preliminary 12‐item checklist was developed and disseminated to a panel of physicians in anesthesiology, emergency medicine, and critical care medicine. A modified Delphi method was used to revise the checklist. After consensus was achieved on a final checklist, recorded video laryngoscopy intubations were evaluated to assess the usability, reliability, and validity of the checklist. Inter-rater reliability was calculated with Cohen’s kappa. Item‐total correlation was calculated for each checklist item to determine discriminatory ability.

Results: The expert panel achieved consensus on a 10‐item checklist after three rounds of revisions. The overall inter‐rater reliability was strong (κ = 0.84) with individual item agreement ranging from moderate to strong (κ = 0.62 to 1.00). Seven of ten items had an item‐total correlation greater than 0.2.

Conclusions: This study details the rigorous development of a novel validated and reliable checklist for the review of video laryngoscopy designed by a multidisciplinary team of airway experts. Checklist scores demonstrated strong inter‐rater reliability. This checklist can be used in future studies and to provide structured education and feedback to medical trainees.
