(1114) Crisis Resource Management Pilot Curriculum for Pediatric Residents

Introduction: Pediatric residency training lacks formal education and practice of team leading during an acute clinical event. Crisis Resource Management (CRM) plays an important role in effective team leading. Physician leadership has downstream impact on the team dynamics, patients and families, and hospitals as institutions. Quality leadership positively affects healthcare delivery. Communication is the responsibility of the team leader, and thoughtful communication allows for better teamwork. Outlined in the principles of CRM, communication translates into clinical practice. Training in team leading has an impact on patient outcomes and satisfaction, provider satisfaction and burnout, and medical education through teaching and role modeling for trainees.

Methods: Implementation of a pilot curriculum consisting of three 1-hour didactic sessions within the Johns Hopkins All Children's pediatric residency didactic schedule. One introductory classroom session introduced the CRM principles and two simulation-based sessions allowed for hands-on practice with these principles. Prior to the first session, a survey developed within Qualtrics was given to the pediatric residents to assess their previous experience with and within crisis/code situations and baseline understanding of CRM principles. Following the pilot curriculum, a similarly developed survey was distributed via Qualtrics to assess the pediatric residents understanding of and experience with CRM principles.

Results: The majority of residents who answered the survey had some experience within code/crisis situations. None of the respondents had received training on CRM principles. Of the responds, one resident had been a leader during a resuscitation experience. The rest had been assigned roles such as chest compressions, recorder, or setting up the monitor. The post-pilot curriculum survey showed improvement in assessment of the CRM principles and involvement during a pediatric code.

Conclusions: Implementation of a pilot curriculum to train pediatric residents on CRM Principles was successful in improving their overall knowledge and understanding of leading a team during a crisis. Through both classroom didactic and hands-on practice, the experience of pediatric trainees in managing a crisis can be enhanced.
